I am sending this request under the Freedom of Information Act to ask for the following information for the period of 2021 – 2024 inclusive: 3. How many complaints about Sildenafil (NHS website: https://www.nhs.uk/medicines/sildenafil-viagra/) have you received through the Yellow Card scheme c. Please, if known, break it down per brand of the product ingested (if Viagra, Aronix,Liberize, Nipatra, Revatio, Grandipam, hims, Kamagra or else) h. Please, indicate if the product used was licensed for use in the UK or not l. Please, if known, indicate the medical condition for which the product was taken for (if erection problems, pulmonary hypertension or else) q. Please also, if known, include from which pharmacy or online site the product came from u. Please Include the year and if it has led to people being hospitalised and/or resulted in death z. Include what the reported symptoms or adverse reactions were aa. Please send me the data requested in the form of an Excel spreadsheet or as a csv file. Please also note that I am only asking about the aggravated data without any personal information attached to the complaints made. The preferred layout of the data would be: Column 1: Complaint Column 2: What product was the Sildenafil contained in = Name Column 3: Licensed for use in the UK = Y/N Column 4: What medical condition was the Sildenafil intended for = Name Column 5: What pharmacy or online site did the Sildenafil come from = Name Column 6: Year of occurred incident = YYYY Column 7: List of Reported Symptoms or adverse reactions Column 8: Led to hospitalisation = Y/N Column 9: Led to death = Y/N If aspects of my request exceed cost limits, I would appreciate any partial disclosure within the limit, prioritising questions a, b, e, f.
